      Inpatient Care Coordinator IV - Full Time, Days (Remote)

      Including mastery of the Inpatient Care Coordinator III role, this position also serves as a lead point-of-contact and backup for all other care coordinators. Managing a reduced case load, they will support the inpatient case managers and care coordinators as necessary to ensure seamless training, workflow, and highest level of care.

      Job Responsibilities/Duties

      • Serve as a lead point-of-contact for all other care coordinators
      • Assist in day-to-day escalation issues and questions from other care coordinators
      • Reviews daily census for case assignments
      • Enters all referral requests and approves referrals that are on the “Approval” list for coordinators.
      • Documents all pertinent information in IDX or Essette.
      • Uploads all Hospital Documents post discharge into IDX or Essette.
      • Manages the inpatient call queue (high volume).
      • Manages work voice mail and responds to messages within 24 hours.
      • Notifies the discharge coordinator of discharges.
      • Discharge planning- call hospitals, finding home health, SNF, DME, specialist and authorize services as appropriate with CM
      • Provides coverage for Inpatient Coordinator III when needed
      • Check fax logs throughout the day, assist as needed
      • Daily log for Claims Review Nurse- generate report and data
      • Analyze and report phone log data for productivity metrics
      • Trains/onboards new care coordinators
      • Provides retraining of existing care coordinators as needed
      • Assists in building and automating training programs
      • Ensures NOMNC Log is accurately completed on a weekly basis
      • Follows up with case managers for corrections to the NOMNC Log needed
      • Monitor the triage rotation when in use
      • Special Projects as assigned
      • Other audits as assigned

      Qualifications

      Minimum Education: High school diploma or equivalent required.

      Minimum Experience: Minimum one to two (1-2) years in the Inpatient Care Coordinator III role or equivalent required. HMO/Medical Group experience preferred.

      Req. Certification/Licensure: Certified Medical Assistant preferred. Medical Terminology certificate preferred.
